Please note that the early registration fee is applied until September 30, 2010.
- Participants, Residents/Students and Accompanying Persons are entitled to Symposium materials, admittance to the scientific sessions and to the exhibition area, all coffee breaks and lunches, and Get Together.
- Accompanying persons mean registered spouses and family members.
- Residents/Students planning to participate in the Symposium are requested to send the Organizing Secretariat a copy of their ID or the signature of their official supervisor at their university or academic institute via fax or e-mail.

Application and Inquiry
Participation is open to all persons interested in basic science and clinical application of Meniere's disease and inner ear disorders. Registration for the Symposium and Application for Social Programs can be made in one of the following ways.

  1. Online registration
    Online registration is available at URL: http://www.acplan.jp/meniere2010/. Please fill in the online registration form and send via Internet.
     
  2. Fax or post-mail of the registration form
    Please download the registration form (PDF) at URL: http://www.acplan.jp/meniere2010/ and send it by fax or post-mail to the Organizing Secretariat, whose address is indicated on the top page.

Payment
All payment should be made in Japanese Yen. You can select one of the following methods for payment of the registration fee. Confirmation of registration will be sent to the registrant by e-mail when payment has been received.

  1. Bank Transfer

    Please make a bank transfer within 10 days after registering. If a remitter's name is different from a registrant's (e.g. university or company name), please notify the Organizing Secretariat.
     
  2. Credit Card
    VISA or Master Card payment is acceptable. Please provide the necessary information and send it via Internet, by fax or post-mail.

Registration Desk
The on-site registration desk will be open at Kyoto International Conference Center during the following hours. Please be advised, however, that advance registration is recommended to facilitate on-site registration during the Symposium.

Cancellation and Refunds
Written cancellation requests must be received before October 15, 2010. In this case, 70% of the registration fee will be refunded after the Symposium.
